Outlines review of systems (ROS), physical exam findings, labs, imaging, differentials, assessment / problem list, and plan. Includes space for to do list and an area for future updates. Each page is perforated so that pages can easily removed if needed. Can be applied to family medicine, internal medicine, hospital rounds, etc. Ideal tool for clinical rotations for medical students, physician assistant students, nursing students, etc. Received an extra copy as a gift during my own  clinical rotations and did not end up using it.
