This is a Quadro P1000 graphics board made by NVIDIA.

As you can see in the 3rd image, the currently attached I/O bracket (the metal part on the left) is for 3 ports.
Originally, this card has 4 Mini DisplayPort outputs, but the top port is hidden.
As it is, you can only output to up to 3 screens. If you want to use all 4 screens, you will need to replace the bracket with a 4-port compatible one yourself or modify it.
